A forehead lift is often performed to treat conditions associated with aging. It may be done in conjunction with other cosmetic procedures to achieve a more harmonious facial appearance. A forehead lift is also appropriate for treatment of certain inherited traits. Younger adults who have a low brow or who already have deep frown lines due to stress or over activity of muscles may benefit from the Forehead Lift procedure.

Am I a good candidate for Forehead Lift surgery?

Any one or combination of the following conditions may indicate that you are a good candidate for a Forehead Lift:

Chin , mandibular and facial implants.

An undersized chin can make the patient’s nose appear more prominent than really is , and the jaw line much weaker.The anatomical chin implants enables more aesthetic harmony with the nose, jaw and other facial features.

Chin augmentation (genioplasty or mentoplasty) can strengthen the appearance of a receding chin by increasing its projection. Simultaneously, it can create better harmony between the chin and other facial features for a more balanced profile. Chin augmentation will not affect your bite or jaw function.

Understanding the Surgery :

There are two techniques for adding prominence and contour to your chin. One, performed through an incision inside the mouth, requires moving the chinbone. The other approach involves insertion of an implant. The incisions to insert the implant may be made inside the mouth or on the underside of the chin. In the latter case, surgery usually leaves a faint scar that is barely visible underneath the chin.

To augment the chin, Dr Milla begins by making an incision in the natural crease line just under the chin or by gently stretching this tissue, Dr Milla creates a space where an implant can be inserted. This implant, made of synthetic material that feels much like natural tissue normally found in the chin, is available in a wide variety of sizes and shapes. This allows custom fitting of the implant to the configurations of the patient's face. After implantation, the surgeon uses fine sutures to close the incision. When the incision is under the chin, the scar is usually imperceptible.

The malar ( facial implants )

Due to midfacial Hipoplasia, a flat malar área can give the patients face tired and sunken appareance, causing other facial features to appear out of balance. The malar ( check implants ) enhancing the facial middle third with a volumetric fulfilling for a more youthful look .

Mandibular implants

Beauty is not an exact science but according to some Plastic Surgeons there is a specific proportion system that the 'ideal' face tends to hover at.  This includes facial height, width and symmetry.  First the face is evaluated from its frontal view and then its lateral (side view).  There are steps that can determine the facial lateral width or projection (as in your profile) and there is the oblique assessment for contours such as the cheek bones, chin and nose. 
Jaw augmentation is the use of synthetics or biologicals to augment, or make the mandibular structure of the face more prominent or defined.  It can give balance to an otherwise less defined face or further augment an already existent jaw structure. To permit proper healing following chin augmentation, you may be placed on a liquid diet for a day or two. The area may be taped or bandaged.

Similarly, in some cases the cheekbones may be built up (augmented) by placing an implant over them. This is usually performed through an incision inside the mouth, but it may be done through a lower eyelid or brow lift incision.

After a chin or cheek augmentation, you will most likely be up and about the same day as surgery and back to work within one or two weeks.

Rhinoplasty is a precise surgery in which the margin of error is measured in millimeters. The nose, like any other projection body structure, is made up of three components: Framework, support and external cover. Dr. Milla is an artist, sculpting the nose cartilage, merging both the functional and the aesthetic aspects of the procedure. The results restore the balance of the nasal structure, improving the visual harmony of the three dimensions of the nose.
Rhinoplasty, or surgery to reshape the nose, can reduce or increase the size of your nose, change the shape of the tip or the bridge, narrow the span of the nostrils, or change the angle between your nose and your upper lip. It may also correct a birth defect or injury, or help some breathing problems.
Rhinoplasty usually takes an hour or two, though complicated procedures may take longer. During surgery the skin of the nose is separated from its supporting framework of bone and cartilage, which is then sculpted to the desired shape. The nature of sculpting will depend on your problem and the surgical technique. Finally, the skin is red raped over the new framework.
This surgical procedure may be performed from within the nose, making the incisions inside the nostrils. In some cases, especially in more complicated ones or in secondary procedures, an “open” procedure is indicated; small incisions are made across the columella, the resulting scars from an open procedure are barely noticeable after a few months after the surgery.

¿Who is a candidate?

Fat grafting is a cosmetic procedure where your own fat is used to rejuvenate the face, augment the cheeks or lips, or correct problems such as hollowness around the eyes or acne scars. Many people lose facial fat as they age and develop a hollow or gaunt look. Fat grafting can be used to correct this and give a younger, more refreshed look. Fat grafting is also referred to as free fat transfer. Some of the fat reimplanted is reabsorbed within a few months, but some of the augmentation will be permanent.

For over a century, plastic surgeons have searched with little success for a filler to restructure the face and body. Physicians have used a bizarre assortment of foreign substances including gold, silver, paraffin, sponge rubber, silk, gutta-percha, and ivory; and more recently, silicone, collagen and Goretex. Despite well publicized complications, some of these remain in common use because we have not found a more suitable filler. With Fat Grafting we can now use the patient's own fat to alter the structure of the face and body.
To ensure lasting changes, the transplanted fat must be able to survive in its new environment. With Fat Grafting, fat is gently harvested, refined with minimal handling, and painstakingly layered into minuscule strands. The method isolates each bit of fat within the surrounding muscle, skin, bone, cartilage or connective tissue. This specific layering method creates a large surface area of contact with the augmented area. This ensures that nutrition can be exchanged through capillaries in the enveloping tissue. This is essential for the fat to survive.

As we age, the effects of gravity, the exposure to the sun, and the stresses of daily life can be seen in our faces. Deep creases form between the nose and mouth; the jaw line tends to sag and fat deposits appear around the neck. Also, the tissues in our face including skin, fat, and muscles tend to thin out as the natural course of aging takes its tole. The facelift can’t stop the aging process. It can give you a more youthful appearance in a variable degree, improving the most visible signs of aging by removing excess fat, tightening underlying muscles, and redraping the skin of your face and neck.
There are constant advances in facelift techniques that help, to a large extent, improve the results that may be obtained. A facelift can be done alone, or along with procedures such as forehead lift, eyelid surgery or nose reshaping.

Many people, especially fair-skinned people in sunny climates, will develop areas of the face as they age where superficial veins and capillaries in the skin become enlarged and highly visible. Because of their typically irregular and spidery appearance, these superficial cutaneous vessels are commonly referred to as 'spider veins'.

The medical term for this phenomenon is 'telangiectasia', which is essentially Greek for 'the stretching out of the very end of a blood vessel'. And that's essentially what 'spider veins' are. They occur most frequently in the central face, particularly on the anterior cheeks and at the base of the nose. Some people develop very prominent spider veins on the dorsum and tip of the nose, where they are difficult to conceal.

Rosacea is a condition of the facial skin where the central face (nose, cheeks, chin, central forehead) gradually turns red and develops numerous fine telangiectasias. These areas easily become flushed, particularly after physical exertion, eating spicy food, or having a glass of wine. Rosacea has traditionally been treated with topical agents and oral antibiotics, however most patients experience improvement only when the medications are in use, with return of rosacea once the treatment is stopped. For many patients, rosacea is fairly resistant to topical and oral medications.

The cause of spider veins and rosacea is not completely understood, and appears to be multi-factorial. Contributing factors may be a genetic predisposition, the chronic use of topical steroid agents, and solar damage to the facial skin that gradually builds up over many years. Rosacea occurs most commonly in warm climates where people get a great deal of year-round sun exposure.

These conditions can perhaps be prevented (in part) by the avoidance of excessive exposure to ultraviolet radiation (the sun, that tanning bed), the avoidance of chronic topical steroid use, and by the daily use of a high-SPF facial sunblock. In some cases, telangiectasias are associated with a systemic autoimmune disorder, such as scleroderma. Once a person develops facial telangiectasias and/or rosacea, these conditions in general do not spontaneously disappear.


So how do I get rid of rosacea, or my facial spider veins, or both?

The ideal treatment for 'spider veins' and rosacea should be easily administered, safe for the patient, reliably effective, should not require an excessive number of treatments, should be non-invasive, and should not have a prolonged recovery period.

Here's the good news: such a vein treatment does exist. A pulsed-dye laser developed specifically for the treatment of cutaneous vascular disorders, can completely eliminate most facial 'spider veins', usually in two to three treatments (large areas of long-standing spider veins may require more treatments), and can reverse the cutaneous manifestations of rosacea as well. In addition, the laser energy pulse delivered by the laser has been modified to limit the development of post-treatment bruising (purpura), a problem associated with older pulsed-dye lasers.

The laser uses light energy of a wavelength that is specifically absorbed by structures which are reddish or purple in color, i.e. capillaries and small arteries and veins. The laser beam incorporates a Dynamic Cooling Device which sprays the skin with a cooling agent immediately before every laser pulse. This advanced technology both protects the skin from thermal energy and reduces the discomfort previously associated with pulsed-dye laser treatment. The laser pulses are generally described by patients as feeling like 'a rubber band snapping against the skin'. No anesthesia (topical or otherwise) is required.

While pulsed-dye laser technology has been available for decades, pulsed-dye laser treatment of rosacea and facial telangiectasia was not frequently performed as the laser energy tended to explode superficial blood vessels, producing facial bruising that may persist for several weeks. The new laser modulates both the peak energy and the duration of the laser pulse in order to coagulate rather than explode dermal blood vessels, which allows treatments to be performed that produce little or no bruising. This advanced technology makes it possible to achieve significant clearance and even complete elimination of facial redness and spider veins with no 'downtime'.


Is Laser therapy a 'cure' for rosacea?

That really depends upon how one defines 'cure'. If by this one means "can you eliminate the facial redness, flushing and spider veins that I walk into your office with?", then the answer is YES - we are able to completely eliminate these findings for many of our patients with a series of laser treatments. However, any individual who has experienced resolution or significant improvement in rosacea following laser treatment is at risk for recurrence of facial redness and flushing if they do not take steps to avoid the primary risk factor: ultraviolet light (sunlight) exposure.
We strongly recommend that our patients 'protect their investment' by using a high-quality, chemical-free sunblock on a daily basis, rain or shine, summer or winter. Hats are also highly recommended in the summer months. Appropriate protection from damaging UV rays can prevent the recurrence of the facial redness and flushing associated with rosacea for most patients.
